Service: Accident Reports (aka Crash Reports)
Description: Copies of crash reports are available in person at any Troop or at the State Police Traffic Records Unit at Headquarters in Baton Rouge. If you want a copy of the report via mail, complete Section I of the form and mail it to the State Police Traffic Records Unit address below. A certified check, company check, or money order for $7.50 must accompany the request. Personal checks and cash will not be accepted.

Please allow ten (10) working days after the crash before submitting a request for a crash report.
