Authorization Form - to be used in conjunction with the appropriate Disclosure Form when requesting background checks

General Disclosure Form - to be submitted when an employer is requesting a background check on an employee

Rap Disclosure Form - is to be submitted when requesting background checks for various boards such as State Board of Medical Examiners, State Board of Nursing, La. Board of Pharmacy, State ABC Board, etc.
