TAKE CHARGE offers Medicaid coverage for family planning waiver services to women ages 19-44 who are Louisiana residents, have an income below 200% of the Federal Poverty level, and do not have health insurance that covers Family Planning services. View the current Federal Poverty Level income guidelines and know whether you qualify.
What does TAKE CHARGE Cover?
The TAKE CHARGE/ Family Planning Waiver program covers services including:
- Four physical examinations or necessary re-visits each year for services rendered by a physician, nurse practitioner, or physician assistant for the purpose of family planning.
- Laboratory tests for the purpose of family planning
- Approved medications and supplies (examples: birth control pills, patches, IUDs, diaphragms, etc.)
Primary care services are not covered by the TAKE CHARGE program. When a need for primary care treatment is identified during a family planning waiver visit, the waiver participant will be given information about primary care providers. A list of available primary care service providers is available here. The patient is responsible for all charges associated with primary care services.
